SAMPLE COLLECTION INSTRUCTIONS

For guaranteed confidentiality and anonymity, follow the "Confidentiality and Anonymity Instructions" contained in this document.

Please follow the instructions below to ensure your samples are collected properly. We recommend you read the instructions through entirely one time before beginning your collection. Only collect one sample at a time.

Pre-Collection Item Checklist

•	2 cotton-tip sterile swabs or regular cotton-tip swabs (e.g., a Q-Tip) per person tested IMPORTANT: If you use a regular cotton-tip swab such as a Q-Tip, cut off one of the cotton-tips so the swab only has one cotton-tip. DO NOT TOUCH THE REMAINING COTTON-TIP WITH YOUR FINGERS. Sterile cotton-tip swabs can be found at your local pharmacy behind the counter and are approximately $0.25 per pack (2 swabs per pack). •	2 small paper envelopes per person tested
•	1 small plastic bag (e.g., Ziploc bag) per person tested
•	1 Sample ID Slip per person tested (located at the end of this document)

Step-by-Step Instructions

1	Wait at least 90 minutes or longer after eating or drinking before swabbing mouth. The best time to collect a swab sample is in the morning before the consumption of food or beverage.
2.	Cut out one Sample ID Slip from the end of this document and fill-in the necessary information about the person being tested.
3.	Place the completed Sample ID Slip in a small plastic bag. This plastic bag will hold the cotton-tip swab after sample collection.
4.	Place swab inside the mouth between the gums and the inside of the cheek. Rub back and forth with a small amount of pressure. Rotate the swab 12-15 times using your thumb and forefinger. Remove from mouth. DO NOT touch the cotton-tip with your fingers.
5.	Allow the swab to dry for approximately 5 minutes.
6.	Place the dry swab inside the paper envelope. DO NOT lick the gummed seal.
7.	Place the paper envelope in the plastic bag with the Sample ID Slip as prepared in Step 3.
8.	Repeat Steps 4-7 with the second swab. IMPORTANT: There should be two (2) swab samples per person tested.
9.	Seal the small plastic bag containing the Sample ID Slip and envelopes so it is air tight (use tape if necessary).

This completes the sample collection.
Repeat all steps per person being tested.

IMPORTANT: For each person tested you should have a small plastic bag containing two envelopes and a Sample ID Slip. Each envelope should contain one swab.

Sample ID Slips

Cut-out the following slips and place each one in a small plastic bag for each person being tested as described in Steps 2 and 3. You may have left over Sample ID Slips.

Confidentiality and Anonymity Instructions: Use a false name or simply "Alleged father 1" or "Child 1" when completing the Sample ID slips and Paternity Testing Form. Make note of the false names you use.
